Absence Declaration — Evaluations

If you need to be absent from an evaluation as part of one of your courses (e.g., a test, midterm, or final exam), you must complete the declaration of absence form*. Supporting documentation must be submitted with all requests except for the first submission in a course for a short-term illness, unless the absence occurs during an examination that has begun. Your request will be evaluated and you will be notified of the decision by email.
* If you have a scheduling conflict for final exams, you must submit your request for a deferred exam by email to your home faculty. Please do not complete the declaration of absence form for exam conflicts.
